Credit Application Instructions
- All MQ Automation Account Setup files are in Adobe PDF format. If you do not have Adobe Reader installed on your computer, please install a free version of Adobe Reader.
- Download the Credit Application
- Complete the appropriate forms and sign the Credit Agreement. Credit Agreement must be signed by an Officer or Owner.
- If applicable, locate your Annual Tax Exemption Certificate to include with your Credit Application. If your Annual Tax Exemption Certificate is not received, your account will be taxed.
- Fax the appropriate forms to 513-351-4395 – Attn: Charlie Shoup. Once received, your information will be processed by an MQ Automation Representative and you will be contacted regarding your account status.
